Description
Promethazine hydrochloride is a first-generation antihistamine of the phenothiazine class with antihistaminic, antiemetic, sedative, and anti-motion sickness properties. It is used in the symptomatic management of selected allergic conditions and nausea-related disorders under the supervision of a qualified healthcare professional.

Indications
Promethazine hydrochloride oral solution is used for the relief of allergic conditions such as seasonal and perennial allergic rhinitis, vasomotor rhinitis, allergic conjunctivitis, uncomplicated allergic skin manifestations, and as adjunctive therapy in anaphylactic reactions after acute symptoms are controlled. It is also used for prevention and control of nausea and vomiting, motion sickness, and for sedation in selected preoperative or postoperative settings.

Dosage & Administration
Dosage should be individualized based on age, indication, and patient response. Promethazine is administered orally, usually in divided doses depending on the condition being treated. Standard references describe varying regimens for allergy, nausea and vomiting, motion sickness, and sedation, so administration should follow the prescribing physician’s instructions and the product labeling. Promethazine is contraindicated in pediatric patients younger than 2 years because of the risk of fatal respiratory depression.

Mechanism of Action
Promethazine acts primarily as an H1-receptor antagonist, reducing the effects of histamine. It also has anticholinergic, sedative, and antiemetic actions, which contribute to its usefulness in allergy management, nausea control, and motion sickness.

Precautions
Promethazine should be used with caution in patients with respiratory compromise, narrow-angle glaucoma, prostatic hypertrophy, bladder-neck obstruction, pyloroduodenal obstruction, seizure disorders, or hepatic impairment. It may impair mental and physical abilities required for hazardous tasks, and concurrent use with alcohol or other central nervous system depressants can increase sedation. It must not be used in children under 2 years of age.

Side Effects
Common adverse effects may include drowsiness, dizziness, dry mouth, blurred vision, constipation, and sedation. More serious reactions may include respiratory depression, hypotension, confusion, extrapyramidal symptoms, and hypersensitivity reactions in susceptible patients.

Storage
Store at controlled room temperature, protected from excessive heat, moisture, and light. Keep the bottle tightly closed and out of reach of children. Storage should follow the manufacturer’s labeled instructions.
